GENERAL DISCLOSURE OF SERVICES
In recognition that millions of Californians receive a substantial volume
of healthcare services from complementary and alternative health care
practitioners, California Law allows access by California residents to
complementary and alternative healthcare practitioners who are not providing
services that require medical training and credentials. The following
disclosure is provided in compliance with Section 2053.6 of the California
Business and Professions Code.
The purpose of a program of hypnotherapy is for vocational and avocational
self-improvement (Business and Professions Code 2908) and as alternative
or complementary treatment to healing arts services licensed by the state.
A hypnotherapist is not a licensed physician or psychologist and hypnotherapy
services are not licensed by the State of California. Services are non-diagnostic
and do not include the practice of medicine, neither should they be considered
as a substitute for licensed medical or psychological services or procedures.

Hypnosis works with the power of the subconscious mind to change habits
and behaviors. The subconscious mind is considered to be the source or
root of many of our behaviors, emotions, attitudes and motivations. Hypnosis
is believed to be a powerful tool for accessing the subconscious mind
and creating improvements in our lives.
Services consist of a program of conditioning, including an undetermined
number of private sessions, depending on the client's individual needs.
The hypnotist will to the best of his or her ability endeavor to accomplish
the objectives of the client's sessions. While hypnosis may be an effective
technique for many purposes, the effectiveness may vary from individual
to individual, and no specific results or progress can be promised or
guaranteed.
During hypnotherapy sessions, clients remain completely aware of everything
that is going on. In fact, many people experience a hyper-awareness where
sensations are perceived enriched and vivid. The ability to visualize
or imagine is enhanced. Deep relaxation is common. Many describe the hypnotic
state as a complete and total escape from physical tension and emotional
stress, while remaining completely alert.
The use of hypnosis could elicit memories of past events which may or
may not be literally true. It is possible that events under hypnosis will
be distorted or misconstrued. Memories or images evoked under hypnosis
are not necessarily accurate and may be a construction or a composite
of memories. Without corroborating information, it is not possible to
determine whether a specific memory is true or false, even if it seems
true to the client.
While it is the practice of Hypnotherapists to keep information confidential,
information revealed in hypnotherapy is not subject to the psychotherapist-patient
privilege. A court may order disclosure of information learned in therapy.

| DISCLOSURE TO CLIENTS
The Hypnosis Motivation Institute (HMI) leases space to HMI graduates
who operate self-employed, private practices offering hypnotherapy services
to the public. The Hypnotherapists who lease office space from HMI are
not HMI employees, but are instead tenants over whom HMI exercises no
control.
Under California law, hypnosis and hypnotic techniques may be used by
individuals not licensed to practice psychology or medicine under two
sets of circumstances: (1) upon referral from a person licensed to practice
medicine, dentistry or psychology, or (2) where the person using the hypnotic
techniques offers avocational, vocational or self-improvement and does
not offer therapy for emotional and mental disorders. (California Business
and Professions Code Section 4996.14; Business and Professions Code Section
2908.)
A hypnotherapist who is also licensed under the Board of Behavioral Sciences
and/or the Board of Psychology can have his/her credentials verified directly
with these boards, both of which have Internet sites.
Sexual, social or business relationships between clients and Hypnotherapists
are unethical, and are strictly forbidden by HMI. If a hypnotherapist
employed by HMI or leasing space from HMI makes any sexual advance or
overture to you, you should refuse to comply and report that individual
to HMI immediately.
The uses of hypnosis may at times elicit memories of past events which
may or may not be literally true.

Payment Policy For Online Orders
Our credit card processor restricts our online payment policy. Our credit
payment processor advice us to put a shipping hold on all orders for at
least 48-hours for payment verification. We cannot accept payment over the
phone or via email. The credit card processors require the customer to enter
all billing related information including but not limited to you billing
name, billing address and charge amount.
Your credit card will show that you are billed for ATOZ Business Computers
by PaySystems.com or 2CheckOut.com. |
Order Online
You can place your order online using our shopping cart buttons and the
secure order form. Please don't send credit card information by email, as
that is not a secure form of transmission. Our credit card processors do
not allow off-line payments.
Online orders will be confirmed via automated email. There will be no
manual confirmation. You may login to your online account to check the
status of your order any time after you placed the order. You will be
notified if an item will not ship within 48 hours due to availability.
Please use care when entering your shipping address. If you enter an
incorrect address and package need to be reshipped, you are responsible
for all cost to reship plus a $5.00 handling fee.
Please make sure your email address and phone number are accurate or
your order may be delayed. Your order will be delayed if your email is
returned undeliverable,.
If we don't have your proper billing address (this is the address your
credit card bill comes to) your order will be delayed or voided.
Order Cancellation
Order can be cancelled only if the order status is "Pending"
by emailing us. You may not cancel the order after the order status has
been changed to "Shipped" or "Completed". You may
not cancel the order if you order is delayed or lost in transit. We have
no control after the package is sent. We will re-ship the order as soon
as we determine the cause of delay of delivery. We will give you full
refund if the order cannot be re-shipped because of availability.

Shipping Policy and Charges
We ship within 3 days after the payment has
been confirmed; this time is needed to process a transaction and to create
a package.
A handling fee of $2.50 will be added to all online orders shipping to
U.S. address.
A handling fee of $5.00 will be added to all online orders shipping to
non-U.S. address.
The UPS and USPS rate are calculated via live connections and is the exact
charge from UPS and USPS
The minimum weight of UPS is one pound. The minimum weight of USPS Priority
Mail is 12 oz
We reserve the right to change and choose other carrier if the shipping
cost and delivery time is not affected.
UNDER NO CIRCUMSTANCES WILL ANY SHIPPING CHARGES OR HANDLING FEE BE
REFUNDED
Insurance
Insurance and confirmation is required to protect package damage or
loss in transit. All packages that are less than $50.00 are self insured
by us. This will speed up damage or lost claim. No one will have to fill
out claim from and wait 60 days for packages below $50.00. Package value
above $50.00 will be insured by the carrier.
Over Night/Next Day Shipment
Please note that our credit card processor requires up to 48 hour wait
on all credit card payment for verification. Therefore if you have choosen
over night or next day delivery, you package may not go out until up to
3 days after your place the order. We ship on the second or third day
after the payment has been confirmed; this time is needed to process a
transaction and to create a package.
Shipping Transit Time
USP Ground - up to 5 days across continental USA
USPS Priority Mail and First Class mail (under 12oz) - one to three days.
Please contact us if you do not recieve your package 10 days after you
recieved the Shipping Confirmation from us.
International Shipping
You are responsible for the law and restrictions of your country on what
can be imported. We are not responsible for any loss due to laws or restrictions
of your country. You are responsible of what you can import to your country.
OUR RESPONSIBLITY STOP FOR ALL INTERNATIONAL ORDER AFTER WE SEND THE PACKAGE
OF THE ORDER TO THE CARRIER. We can only provide you with proof of mailing
or tracking number for your package. Shipping insurance may not be available
for some countries. You are responsible for any loss if shipping insurance
is not available for your country. We are not responisble for packages
opened, damaged or confiscated at the Custom of your country.
Please Contact Us for International Shipping. We ship to most European
countries, Australia, New Zealand and most Asian Countries. Our credit
card processor restrict where we can accept International credit payment.
We do not ship to the following countries due to either international
law or the high rate of credit card fraud we have experienced. Afghanistan,
Cuba, Iran, Iraq, North Korea, Libya, Serbia, Sudan, Romania, Indonesia,
Singapore, Hungary, Belarus, Estonia, Latvia, Lithuania, Slovak Republic,
Ukraine, Russia, Yugoslavia, Macedonia, Philippines, Thailand, Malaysia,
Pakistan, India.
All International shipping is at our discretion and we reserve the right
to decline any order or any portion of an order to any country:
Custom Clearing
Some countries take a very long time for packages to clear custom. Some
of our customer has experienced custom clearing time of over 30 day. There
is nothing we can do to speed up the process. We have no control after
we send your International order. We can only provide you with proof of
mailing or tracking number for your package. We are not responisble for
packages opened or confiscated at the Custom of your country.

Return Policy
Please read the description of the item carefully. Please make any inquiry
before your place the order. All sales are final. Items are returnable
only if we ship the wrong item or if the item was defective. Wrong items
will be replace. Defective items will be replaced or repaired at our discretion.
Please be sure to contact us if your have any problem. We will do our
best to have the problem resolved. We can only credit you the same way
you pay.
Defective Item Return Authorization
Please contact us for a Defective Item Return Authorization Number for
defective items.
Shipping Defective Item to Us
Please return defective items insured and pack the defective item carefully.
It is important that you follow our Return Shipping Instruction. We reimburse
you the return shipping cost per our Return Shipping Instruction. We cannot
reimburse you anything other than the shipping and insurance cost according
to our Return Shipping Instruction. If you choose to ship the defective
item to us any other way, you will be response for any extra cost.

Warranty
We offer a 90 day warranty on material and workmanship for everything
that is manufacture by us. Please refer to our return policy for defective
items. Product manufacturers carry their own manufacturer's warranty.
We will provide all documentation necessary to make manufacturer's warranty
claim.

Privacy Policy
Password and User IDs
Your user ID and password, should be maintained as confidential and should
not be shared with anyone or distributed or disclosed to third parties.
It is your responsibility to notify us if you need to change or discontinue
the user ID and password and you may do so by contacting by phone, fax
or via email. It is also your responsibility to request such discontinuation
immediately upon your knowledge or belief that such user ID and/or password
is or may be subject to a breach of confidentiality.
Cookies
You should be aware that non-personal information and data may be
automatically collected through the standard operation of our Internet
servers or through the use of "cookies." Cookies are small text
files a Web Site can use to recognize repeat users. We may use cookies
to recognize visitors and more quickly provide personalized content or
grant you unimpeded access to registered areas of our web site.
Information gathered through cookies also helps us measure use of our
site. Cookie data allow us to track usage behavior and compile data that
we use to improve the site and target advertising. This data will be used
in aggregate form; no specific users will be tracked. We do not use cookies
to uniquely identify you, except where such use helps us to make your
visit more efficient. Cookies help you travel through our site efficiently.
Our Security Policy
We are awared of your privacy concerns and strive to collect only
as much data as is required to make your Internet and/or product or service
use experience as efficient and satisfying as possible, in the most unobtrusive
manner as possible.
The foregoing policies are effective as of January 1, 2005. We reserve
the right to change this policy at any time by notifying users of the
existence of a new privacy statement. This statement and the policies
